At a McDonald's in Arkansas the manager had to explain to the person
working the register that cheese was standard equipment on a Quarter
Pounder with Cheese (tm) and that they didn't have to press the cheese
button.

At a different McDonald's also in Arkansas on another trip just before
putting the burger in the bag the burger assembler realized they forgot
to put the cheese on it and had to disassemble it to add the cheese.
