Junk in, junk out.

1) Your "survey" looks like you sat down and asked the questions you
were interested in without doing any preliminary research-- resulting
in questions that fit your own preconceptions, but perhaps not
aligning with the categories that really matter to your respondents.

2) Your "survey" does not give options of "don't know," "other," or
"not applicable," yet insists that every question be answered. This
is going to skew up your results big time.

3) Your "survey" does not seem to allow for the possibility of your
respondents to write text in response to any of your questions--
though I don't know: I couldn't get past your first page because I
hadn't answered every question.

I expect your "survey" is going to get a very low response rate from
any (potential) respondent who understands the situation differently
from you: they will quit the "survey" before they have finished the
first page. Because (I'm guessing) you have set the terms without
preliminary research, you are going to "find out" exactly what you
believed when you wrote the questions.
