Default Salmonella in the peanut butter?

No, peanuts like potatoes are grown in the ground.
They undergo a preliminary drying in the field
(during which it may rain, encouraging microbial
growth), followed by further drying in sheds.

Peanut butter is made from the lowest grade of
peanuts allowed for food use. Horrible peanuts,
many with broken shells which further increase the
risk of contamination.

It's not surprising that batches would be contaminated
from time to time.
