The usual I find is a burger patty, swiss cheese, and grilled onion on rye
bread that is then grilled on both sides until the cheese is nice melted.

The worst I've had has usually included the same ingredients, just not well
executed. I've never been served one made with American cheese, but quality
of execution has varied.
